Forms requesting funds from awarded grants

Hello Teachers!

Congratulations on being awarded a grant! To request funds, you need to fill out a Grant Request Form and email it to Jennifer Schultz at schuje@centennialsd.org

Please request the funds 2 weeks prior to when they are needed, so we allow time to review the request and get the funds ready for your mini-grant project. The form can be downloaded via the link below.
